چکیده

Results At first evaluation he had arthritis of both knees, left ankle and 4th MTF of the left foot and tenosynovitis of second finger of the right hand. His immune evaluation revealed an important CD4 naif lymphopenia, abnormal proliferative responses to mitogens, skewed Vbeta repertoire and expanded gammadelta T cells, as well as conserved humoral response. Laboratory investigations showed erythrocyte sedimentation rate 25 mm/hr, Creactive protein 14.5 mg/l, antinuclear antibodies positive (1/80), anti-cyclic citrullinated protein antibodies and rheumatoid factor were negative. A JIA-like disease was assumed, he started ibuprofen (30 mg/kg/day), and a chemical synovectomy with triamcinolone hexacetonide (TH) was performed (knees and ankle). Ten months after diagnosis, knees and left ankle remain without active synovitis, but active 4th MTP of left foot and tenosynovitis of 2nd finger of right hand persists despite moderate improvement. Taking in account the underlying immune defect methotrexate was not an option and hydroxichloroquine (5 mg/kg/day) was started.

Autoimmune Hemolytic Anemia in a Patient with Probable Ataxia Telangiectasia: A Case Report

Background: Ataxia telangiectasia (AT) is one of the combined immunodeficiency syndromes with immunologic, neurologic, endocrinologic, hepatic and cutaneous abnormalities. Regarding the fact that autoimmune disorders; such as autoimmune hemolytic anemia (AIHA), are not generally expected in the course of AT, we present a patient with an unusual presentation of these two conditions. Case present...

Leukoencephalopathy after prophylactic radiation for leukaemia in ataxia telangiectasia.

Children with ataxia telangiectasia have a high probability of developing acute lymphoblastic leukaemia, and have increased sensitivity to chemotherapy and irradiation. We report a 51/2 year old boy who had undiagnosed ataxia telangiectasia when he presented with acute lymphoblastic leukaemia. He subsequently developed a chemoradiation induced leukoencephalopathy after conventional central nerv...

Epstein-Barr-virus-carrying lymphoma in a patient with ataxia-telangiectasia.

An undifferentiated lymphocytic lymphoma of mesenteric lymph nodes occurred in a young boy with ataxia-telangiectasia. Two independent tests, Epstein-Barr virus (EBV)-cRNA/DNA hybridisation and EBV DNA/DNA reassociation kinetic analysis, showed 53 and 68 EBV genome equivalents per cell respectively, which was compatible with an EBV-genome-carrying tumour. Whether this was a polyclonal lymphopro...
